To complain or make a request for information to the BC Ombudsperson, use one of the forms below. Before you fill out the form, we strongly encourage you to read the information provided on this web site regarding what the Ombudsperson can investigate. Once the the Office of the Ombudsperson receives the complaint form, you will be contacted within five working days by telephone, fax, or by Canada Post. The Ombudsperson will not reply by e-mail.
